An Incident is:

...an unplanned event or circumstance that could have, or did, lead to unintended and/or unnecessary harm. It can include any event that may give rise to physical, emotional or psychological harm.

…Feedback is: An expression of satisfaction or dissatisfaction or acknowledgement of the level and/or quality of the service provided.

Promptly respond to any incidents or feedback that comes to their attention.


Promptly notify your manager of all incidents and feedback and lodge the report in RiskMan

Participate in investigation and review of incidents and feedback as required.

All entries will be investigated to ensure the event contributes to quality improvement.
